Honoring Choices Minnesota Updates Advance Care Planning Forms

[MMA News Now, July 31, 2014] Honoring Choices Minnesota (HCM) recently updated its health care directives to be more user-friendly and has added a new “short form,” which provides additional options for individuals when making their advance care decisions.

The new updates are designed to help physicians and other healthcare providers meet their patients’ needs in advance care planning, said HCM Medical Director Ken Kephart, M.D. The addition of the short form gives flexibility in working with patients of all ages and life experiences, to help meet HCM’s goal of every Minnesota adult completing a health care directive well in advance of it being needed.

The directives are available for free downloading on the Twin Cities Medical Society (TCMS) website. There is also an online resource that explains the updates and changes.   

Honoring Choices Minnesota is an initiative of the TCMS, and is led by a cooperative team of representatives from Minnesota health care systems. For more information, email Program Manager Karen Peterson or call 612-362-3704.
