Emerging Topics in Healthcare, Health Equity and Well-Being

Join your colleagues for MMA’s monthly Physician Forums – opportunities to stay updated on the latest news, information, and resources on important topics affecting the practice of medicine.  
 
Each forum will begin with a short presentation, with open discussion to follow. In this series, you will hear from experts on a variety of topics and engage with your peers from across the state of Minnesota. 
 
Members have free access to these forums; non-members will be charged a fee of $20. 
 
Once you have completed the registration process, you will receive a receipt email from mma@mnmed.org. The Zoom meeting link will be included in that email.

Changing the Culture and Systems of Health Care to Improve Serious Illness Care Planning

The objectives for the forum, which will be led by Erik Fromme, MD, MCR FAAHPM, lead faculty from Ariadne Labs, founded by Atul Gawande, MD, in 2012, include: 

  • Describe the state of serious illness care planning in health systems
  • Summarize proposed best practices for serious illness care planning and the role of palliative care clinicians
  • Describe the tools, training approach, and systems change components of the Serious Illness Care Program (SICP)
  • Review evidence of the impact of the SICP on outcomes of patients with serious illness
  • Demonstrate serious illness conversation using the guide and debrief

The world-renowned and research-based Serious Illness Care program is currently used by dozens of health systems across the country.  Fromme will be joined by Vic Sandler, MD, a geriatrician and hospice and palliative care specialist with HealthPartners, who serves on the advisory council for MMA's Center for Advancing Serious Illness Communication (CASIC).  For more information, please visit www.advancingsic.org.
